We believe in transparency, education, and partnership, and we know that choosing a healthcare provider is an important decision. Direct Primary Care is a membership-based model of healthcare where patients pay a flat monthly fee for access to their medical provider. This model removes insurance from the exam room, allowing for longer visits, better access, and care that is guided by your needs rather than billing requirements.

DPC is not insurance. Insurance is designed to cover large, unexpected medical events, while DPC focuses on day-to-day medical care. In traditional insurance-based practices, visits are often brief and limited by what can be billed. With DPC, care is relationship-based, proactive, and focused on problem-solving without co-pays or surprise bills.

Many patients choose to pair DPC with a high-deductible insurance plan for major medical needs.

Lineage Wellness is a Direct Primary Care practice created by the Trivida team to provide accessible, relationship-based medical care. While Trivida Functional Medicine offers deep, highly individualized programs for complex or multi-system concerns, Lineage allows patients to continue care long term or focus on one issue at a time within a membership model.

Lineage Wellness is not an on-call emergency service. That said, our care model is designed to reduce urgent issues through proactive planning, continuity, and accessibility during business hours. Many concerns can be addressed before they escalate.

For urgent or life-threatening emergencies, patients should always call 911 or go to the nearest emergency room.

Yes. Lineage Wellness partners with organizations to offer Direct Primary Care memberships as an employee benefit. These plans support employee health, reduce unnecessary healthcare costs, and improve access to meaningful care.

Yes. Children under 18 may enroll for acute care only. Please note that we are not a pediatric practice and do not carry or administer childhood immunizations. For routine pediatric care and vaccines, we recommend maintaining a relationship with a pediatrician.

Memberships are month to month. You may cancel at any time with 30 days’ notice.

Lineage Wellness offers onsite phlebotomy and access to highly competitive cash-based lab testing. We can also order labs through LabCorp and Quest when appropriate.

Yes. When specialty care is needed, we will provide referrals and help coordinate next steps to ensure continuity of care.

No. Lineage Wellness does not contract with or bill conventional insurance plans.

No. Lineage Wellness does not participate in Medicare.

There are two main reasons: experience and cost control.

Most insurance plans, especially high-deductible plans, require patients to pay out of pocket for everyday care until the deductible is met. This often means paying higher prices for brief visits, urgent care, or emergency room services, even for non-emergent issues.

With Lineage Wellness, your membership covers the majority of your primary care needs with no co-pays. Increased access and continuity often prevent unnecessary urgent care or ER visits, helping patients avoid reaching their deductible altogether.

Many patients choose Lineage because they want a different kind of relationship with their provider. One that prioritizes time, access, clarity, and proactive care rather than reactive, fragmented visits.
