
HOW WE HELP OUR PATIENTS
Lineage Wellness Memberships
Healthcare should feel steady, accessible, and aligned with your current season of health. Our membership model allows you to choose the level of support that fits your needs now, while giving you the flexibility to adjust as life changes.
All Lineage memberships include relationship-based medical care without co-pays or billing restrictions. We work directly for you, which allows us to focus on thoughtful, proactive care rather than rushed visits.
Every Lineage member receives:
• Prompt appointment availability during scheduled clinic days
• Direct messaging access during business hours
• Phone check-ins when clinically appropriate
• Provider oversight even on non-clinic days for urgent questions when possible
• Onsite phlebotomy
• Access to highly competitive cash-based labs
• The ability to order labs through LabCorp and Quest
• Imaging orders such as X-ray or MRI when clinically indicated
• Guidance on appropriate testing available through local pharmacies or grocery-based clinics when helpful
• Coordination with specialists when referrals are needed
While we are not an urgent care or emergency facility, our model is designed to address concerns early and reduce unnecessary urgent care and emergency room visits whenever possible.

What do we mean by acute care?
Acute care refers to new or short-term medical concerns that arise unexpectedly. This may include:
• Respiratory illnesses, sore throat, sinus infections, or flu-like symptoms
• Urinary tract infections
• Minor injuries or musculoskeletal concerns
Rashes or skin conditions
• Medication questions or adjustments
• New symptoms that require evaluation
If an injury occurs, we can evaluate you and order imaging such as X-rays or MRIs when appropriate. We help guide next steps and coordinate follow-up, even if imaging is performed outside our office.
Acute care is focused, efficient, and designed to help you address one issue at a time while maintaining continuity with a provider who knows your history.
BRONZE MEMBERSHIP
$129 per month
Founding rate for current clients $100 per month
Bronze Membership includes:
- Full acute care access
- Two extensive functional lab review visits per year
Bronze is ideal for patients who have completed a comprehensive functional medicine program and feel stable in their health. It provides maintenance-level support, ongoing oversight, and deeper data review when needed without intensive monthly management.
SILVER MEMBERSHIP
$239 per month
Founding rate for current clients $200 per month
Silver Membership includes:
- Full acute care access
- Four in-depth functional lab review visits per year
- 10 percent discount on supplements
Silver is well suited for patients who are re-establishing care, optimizing hormones or thyroid function, or navigating autoimmune patterns that require more consistent review and adjustment. It offers greater structure and accountability throughout the year.
GOLD MEMBERSHIP
$329 per month
Founding rate for current clients $289 per month
Gold Membership includes:
- Full acute care access
- Six comprehensive functional lab review visits per year
- 15 percent discount on supplements
Gold is designed for patients who want the highest level of access and oversight. It is ideal for those navigating recurring or complex symptoms that require a more concentrated and closely monitored approach.
A note on functional lab reviews
Functional lab reviews are dedicated visits focused on deep analysis of advanced testing. These sessions allow time to interpret patterns, assess trends over time, and thoughtfully adjust your care plan. They go beyond routine lab discussion and are structured for meaningful clinical insight.
Flexible and Transparent
Memberships are month to month and may be canceled at any time with 30 days’ notice. There is a $200 initiation fee for new patients. This covers onboarding, record review, and establishing care. If a membership is canceled and reinstated within the same calendar year, a reinstatement fee will apply.
Lineage Wellness is designed as an ongoing relationship. Active membership is required for prescription refills, medical messaging, and continued care. If you are not actively enrolled, we are unable to refill medications or provide medical management.
We do not bill insurance, which allows us to practice without coding limitations and focus fully on your care.
